The first step to getting started is to contact us for a lab script prescription.

PPO insurance - Take this lab script to a quest diagnostics near you to have your blood drawn. (You can find the location closest to you by calling 1-800-377-8448 or by going to questdiagnostics.com).  Make sure that you do not eat or drink anything (except water) 6 hours before your blood is drawn.  It usually takes about 7-10 days to receive the lab results.  We will call you once we receive the lab results.

HMO insurance - You will need to take the lab script to your “in network” primary care physician for him/her to write the lab script for you in order to be covered by your insurance.  Your doctor simply needs to write a prescription for the lab tests located in the rectangular box on the Genesis lab script.  He will then send you to a lab which your insurance is affiliated with.  Please make sure to ask for a copy of the lab results and fax them to 877-888-2535.

No insurance - Download and print out the lab script which says no insurance.  Take this lab script to local lab/hospital.  Please make sure to ask for a copy of the lab results and fax them to 877-888-2535.
